SAP Referral Form

The SAP Program

The Student Assistance Program is an intervention program staffed by teachers, school counselors, nurses, administration, and appropriate agencies from the community. The "SAP Team" is trained to identify and refer "high risk" students for appropriate evaluation.

The SAP team will help you find services and assistance within the school, and if needed, in the community. We do not diagnose, treat, or refer your child for treatment. We will provide you with information and you make the choice(s) that best fit your needs and wishes. As the parent/guardian you are an important part of the team.

REASONS FOR REFERRALS :Social/Emotional, Academic, Behavior, and/or Drug and Alcohol Referrals for high-risk students come from students, parents, teachers, staff, counselors, administration, and community resources.
